The challenge

Despite strong foundations, leaders recognised an opportunity to evolve the model to deliver stronger outcomes, a better patient experience, and improved cost efficiency.


There was clear board-level appetite to pursue meaningful transformation and a desire to create better long-term value for the organisation funding this unique healthcare ecosystem.


Our Approach


Together we:

  • Developed a board education and engagement programme to build understanding, alignment, and support for the transformation.
  • Facilitated cross-departmental collaboration to design a client-specific ACO model and secure board approval.
  • Supported an organisation-wide change programme, including the rebrand and launch of Care+.
  • Combined structured programme management with an agile delivery approach, enabling six enabler workstreams to progress quickly and deliver early impact across six patient archetypes.
